Section 2: Vital Documents

We need complete up-to-date records, in the event of your death, to determine the benefits to which your dependants or estate may be entitled (see Section 12). If you have not already submitted the following documents to your personnel office or to the Superannuation Sector, please forward them to us as soon as possible:

  1. your marriage certificate;
  2. the birth certificate of any children under 18 years of age, or of children under 25 who may qualify for student allowances (see Section 12); and
  3. any document that affects the status of your dependants, such as a death certificate, separation agreement, or divorce decree.

Photocopies are acceptable and each document should be identified with your superannuation number.

If you submit original documents, they will be returned upon your request.

If you are currently widowed, separated, or divorced, let us know, as this may affect benefits payable in the event of your death. We should be kept informed of any subsequent change in your marital status.

Under certain circumstances, payment of survivor benefits may be made to a common-law or same-sex partner. To determine eligibility for this, evidence must be presented by the claimant demonstrating that he or she had lived with the plan member in a relationship of a conjugal nature since before retirement and for a minimum of one year. If the plan member has provided information in this regard to the Superannuation Sector from time to time, it will be much easier for the survivor to establish his or her eligibility (see Section 12).

If you get married after your retirement, you should submit a copy of your marriage certificate to the Superannuation Sector. You are eligible to elect within one year from the date of your marriage, or from the date your pension commences if later, to provide survivor benefit coverage for your spouse (see Section 12).
