This publication provides standardized descriptions of the health-related quality of life associated with the main cancers that affect Canadians. It documents the functional limitations (physical, emotional and social) experienced by patients at the time of diagnosis of cancer, and as they undergo the most common therapies, such as surgery, radiotherapy, chemotherapy and hormonal therapy. It also assesses the long-term impact of these treatments, as well as the functional limitations during palliative and terminal care. For each phase, from initial diagnosis to remission or palliation, the study describes a "typical" case based on review of literature and clinical experience. Although experiences vary from individual to individual, these descriptions are needed to measure health at the population level. This article describes a workbook system used to integrate the information required for calculating HALYs, YLLs, and YERFs, for 25 types of cancer as well as PAFs for five risk factors. It describes the data sources, calculations, and software used for these estimates. Sample workbook: HALYs for breast cancer Information about mortality and morbidity across health states related to a disease is synthesized in structured workbooks that calculate health-adjusted life years lost for each type of cancer. Users can change various parameters such as population structure and discount rates. The workbooks, one for each type of cancer, are self-contained and can be used directly to explore the details for that type of cancer. Information in them has been drawn from linked databases. These data include population structure, mortality, incidence, distributions of cases entering each health state, durations of time in each health state, preference scores that weight for severity, life table estimates of life expectancies, and risk factor prevalence and relative risks. The entire workbook system is available as a free, downloadable package that runs on a personal computer. The tools are Excel files with embedded macro programs. The downloadable zip file contains the database files initialized with Canadian data for cancers, but data for other countries or specific provinces can be inserted. The package contains the tools, templates and workbooks that estimate HALY, YLL, YERF, and PAF and a user guide. The workbooks that estimate HALY are generated from the system at a rate of approximately one minute per disease.
