How should you tell the CRA about your change of name?

(We do not accept changes of name via the Internet.)

If you change your name, please inform us as soon as possible.

By mail or fax

Send your request by mail or by fax to your tax centre. Be sure to include your:

• signature
• social insurance number
• old and new name
• the effective date

Contact us at your tax services office or tax centre.

If you change your first name or your surname, we will accept the request without documentation.

If you change your first name and your surname, please send a copy of the official change of name document along with your request.
