Instructions for Health Care Associations and Providers

The following sample poster and brochure have been created to help you meet the PIPEDA requirement of informing your patients and clients of the purpose for the collection of their personal health information and how it will be used and disclosed. These tools have been developed in consultation with the Office of the Privacy Commissioner and major health care associations.

The sample poster and brochure can be used in their current format, modified or reproduced to meet your specific needs. A section has been included to allow you to insert your contact information for clients and patients who may have additional questions or require further information regarding their privacy rights.

For your convenience, the sample poster and brochure are also available in French and varying design and file formats.
