Prescribed Forms under the Minor Injury Regulation - December 4, 2007
The following prescribed forms under the Minor Injury Regulation have been updated. Use these prescribed forms for motor vehicle accidents that occur on or after January 1, 2008:
- MI-1: Request for an Assessment by a Certified Examiner Notice Form - effective January 1, 2008
- MI-2: Application to the Superintendent of Insurance to select a Certified Examiner Form - effective January 1, 2008
The previous forms will still be available on this website until January 2008. Use these prescribed forms for motor vehicle accidents that occur up to and including December 31, 2007:
- MI-1: Request for an Assessment by a Certified Examiner Notice Form - effective up to December 31, 2007
- MI-2: Application to the Superintendent of Insurance to select a Certified Examiner Form - effective up to December 31, 2007

Effective November 1, 2007:
- Annual Industry-Wide Adjustment of Rates for Basic Coverage: Bulletin 1-07 and Order 1-07
- Geographical Differential Adjustment to the Grid Base Premium Rates for Basic Coverage: Bulletin 2-07 and Order 2-07
