Have you returned your Registration of Live Birth form?

To receive a birth certificate for your child, you are required to complete and return the Registration of Live Birth form within 30 days of your child's birth to the Vital Statistics Agency. Filing this form is required under the Vital Statistics Act. Birth certificates are important and used for identification and to access benefit programs and services.
