Help on Congratulatory Message Service Request Form
The MLA of the recipient will automatically be notified by the Protocol Office. Upon request, the Protocol Office will also notify the following offices requesting a congratulatory message: The Queen; Governor General; Prime Minister; Lieutenant Governor; and Member of Parliament. MESSAGE FROM THE QUEEN You must provide one of the following in order to obtain a message from The Queen:
For a 100th (and over) Birthday: - photocopy of birth certificate OR - certification of the date of birth by: a priest, minister, rabbi - by Income Security - a notary public OR - the individuals Social Insurance Number
For a 60th (and over) Anniversary: - photocopy of the marriage certificate OR - certification of the marriage by: a priest, minister, rabbi or by the provincial Registrar's Office - a notary public OR - an official document or a dated newspaper clipping of a previous wedding anniversary
* Queens messages are mailed NO EARLIER THAN TWO WEEKS PRIOR to the occasion date. * In order to receive The Queen's message ON TIME FOR THE DAY OF THE ANNIVERSARY OR BIRTHDAY, request and PROOF of birth or marriage must be received at the Protocol Office AT LEAST EIGHT (8) WEEKS PRIOR to the occasion date *** If proof is not provided , a message from the Queen WILL NOT BE ISSUED.
Please ensure that all information is correct. If the information is incorrect, it may not be possible for the five other offices listed above, to issue another congratulatory message.
NOTE: The Queen, Governor General, Prime Minister and Lieutenant Governor send greetings at five year intervals only. To ensure timely delivery, please submit certificate request at least 6 weeks in advance of celebration. Requests for congratulatory messages from the Governor General and Queen should be submitted at least 8 weeks in advance of celebration. No certificates will be issued for requests that are received 4 weeks past the celebration date unless there are extenuating circumstances, i.e. recipients are very ill.
