This form, when signed by a client, allows ISP to determine whether a client is disabled or continues to be disabled under the terms of the Canada Pension Plan.

Deduction and Payment of Canada Pension Plan (CPP) Disability Benefits to an Administrator of a Disability Income Program

ISP has agreements with 36 long-term disability insurers which allow, with client consent, reimbursement of their retroactive payments when their application for CPP disability benefits has been approved.This form is applicable only to private insurance companies which have agreements with CPP. Private insurance companies that would like to have more information may call (613) 954-4584.

ISP has agreements with provincial and municipal Social Insurance programs and 36 long-term disability insurers which allow, with client consent, the release of information regarding a client's CPP account. Companies that do not have an agreement with CPP may call (613) 954-4584 for more information.
