About the consultations

The Ministry of Long-Term Care (MLTC) conducts ongoing consultations to gather public input before deciding on a licensing proposal for a long-term care home. Licensing transactions may include, for example, transferring a licence to a new owner or location, or issuing a new licence post-redevelopment.

These consultations are for anyone — including long-term care home residents, families, staff and other community members — to provide their opinions on a proposed licensing transaction relating to a long-term care home.

As part of the ministry's review, the Director under the Long-Term Care Homes Act, 2007 will consider submissions before making a final decision.

Development of a 128-bed long-term care home in Tavistock

About the proposal

The Ministry of Long-Term Care is reviewing a proposal from peopleCare Communities Inc. (the licensee) for issuing a long-term care (LTC) home licence for peopleCare Tavistock (the home), a 100-bed long-term care home located at 28 William Street S in Tavistock, Ontario.

The proposal involves:

  • the redevelopment of the home’s 100-beds with an additional 28 beds approved by the Ministry of Long-Term Care to create a new 128-bed long-term care home located at a site adjacent to the current home in Tavistock, Ontario
  • the issuance of a new long-term care licence for a new 128-bed long-term care home with a term of up to 30 years

Oakwood Manor - Project #21-028

Development of a 288-bed long-term care home in Niagara Falls

Closing date: August 2, 2021

About the proposal

The Ministry of Long-Term Care is reviewing a proposal from Maryban Holdings Ltd. (the licensee) for issuing a long-term care (LTC) home licence for Oakwood Park Lodge (the home), a proposed 288-bed long-term care home to be developed in Niagara Falls, Ontario.

The proposal involves:

  • the upgrade of 153 beds from Oakwood Park Lodge located at 6747 Oakwood Drive in Niagara Falls, in the current location and renaming the home to Oakwood Manor
  • the transfer of all 65 beds to the Home from Valley Park Lodge located in Niagara Falls and licensed to 955464 Ontario Limited, an affiliate of the licensee
  • the development of 70 additional long-term care beds conditionally allocated by the Ministry of Long-Term Care at the home
  • the issuance of a new long-term care licence for a new 288-bed long-term care home with a term of up to 30 years
  • the closure of Valley Park Lodge following the development of the home

Extendicare Sudbury - Project #21-029

Development of a 320-bed long-term care home in Sudbury

About the proposal

The Ministry of Long-Term Care is reviewing a proposal from Extendicare (Canada) Inc. (the licensee) for issuing a new long-term care (LTC) home licence for Extendicare Sudbury #2 (the new home), a proposed new long-term care home to be developed at Bancroft Drive and Nottingham Avenue in Sudbury, Ontario.

The proposal involves:

  • the transfer and upgrade of 266 of the 288 beds from Extendicare York located at 333 York Street in Sudbury, Ontario to the new home
  • the development of 54 additional long-term care beds conditionally allocated by the Ministry of Long-Term Care to be included in the proposed 320-bed development project
  • the approval of the bed transfers and the issuance of a new licence with a term of up to 30 years for the operation of 320 beds, following the development of the new home
  • the closure of 22 of the 288 beds at the Extendicare York following the development at Extendicare Sudbury #1
  • the closure of Extendicare York following the development of the new home

The Ministry of Long-Term Care is reviewing a proposal from CVH (No. 6) LP by its general partners, Southbridge Health Care GP Inc. and Southbridge Care Homes (a limited partnership, by its general partner, Southbridge Care Homes Inc.) (the proposed licensee) for issuing a new long-term care (LTC) home licence for Southbridge Port Hope (the new home), a proposed new long-term care home to be developed at 20 Hope Street South in Port Hope, Ontario.

The proposal involves:

  • The redevelopment and transfer of 97 beds from Hope Street Terrace and 60 beds from Regency Manor to the new home.
  • The development of 35 additional long-term care beds conditionally allocated by the ministry to be included in the proposed 192-bed development project, subject to project approval and meeting all licensing requirements under the Long-Term Care Homes Act, 2007 (the act).
  • The issuance of a new licence with a term of up to 30 years for the operation of 192 new beds at the new home in Port Hope, Ontario, following the development.

The Ministry of Long-Term Care is reviewing a proposal from CVH (No. 6) LP by its general partners, Southbridge Health Care GP Inc. and Southbridge Care Homes (a limited partnership, by its general partner, Southbridge Care Homes Inc.) (the licensee) for issuing a new long-term care (LTC) home licence to Southbridge Pickering (Orchard Villa), a 233-bed long-term care home located at 1955 Valley Farm Road in Pickering, Ontario.

The proposal involves:

  • The development of 87 additional long-term care beds conditionally allocated by the Ministry of Long-Term Care (the ministry) to be included in the proposed 320-bed development project, subject to project approval and meeting all licensing requirements under the Long-Term Care Homes Act, 2007 (the act).
  • The issuance of a new licence with a term of up to 30 years for the operation of 320 new beds at the existing location, following the development.

The Ministry of Long-Term Care is reviewing a proposal from Carefirst Seniors and Community Services Association (the proposed licensee) for issuing a new long-term care (LTC) home licence for Carefirst Campus of Care (the home).

The proposal involves:

  • The development of 120 long-term care beds conditionally allocated by the Ministry of Long-Term Care (the ministry) to be located at 9893 Leslie Street in Richmond Hill, Ontario, subject to project approval and meeting all licensing requirements under the Long-Term Care Homes Act, 2007 (the act).
  • The issuance of a new licence with a term of up to 30 years for the operation of 120 new beds, following the development of the home.

The Ministry of Long-Term Care is reviewing a proposal from The Corporation of the City of Timmins (the “operator”) for issuing a new long-term care (LTC) home approval for Golden Manor (the “home”), a 177-bed long-term care home located at 481 Melrose Boulevard in Timmins, Ontario.

The proposal involves:

  • the upgrade of the home’s 177 beds on a site next to the home
  • the development of 15 additional long-term care beds conditionally allocated by the Ministry of Long-Term Care (the “ministry”) to be included in the proposed 192-bed development project, subject to project approval and meeting all licensing requirements under the Long-Term Care Homes Act, 2007 (the “act”)

Rainy River Health Care - Project #21-023

Issuance of a new long-term care licence to Rainy River Health Care

About the proposal

The Ministry of Long-Term Care is reviewing a proposal to issue a new long-term care (LTC) licence to Riverside Health Care Facilities Inc. (the licensee) for Rainy River Health Centre (the home), a 21-bed long-term care home located at 114 Fourth Street in Rainy River, Ontario. The current licence expires on June 23, 2024.

At least three years prior to expiration, the Ministry of Long-Term Care must provide notice to the licensee that no new licence will be issued for the home upon expiration or give an undertaking that a new licence will be issued to the licensee if specified conditions are met.

If approved, a new licence for 21-beds with a term of 10 years as of June 24, 2024, would be issued for the home, subject to all applicable requirements and conditions.

Grey Gables Home for the Aged - Project #21-018

Development of a 128-bed long-term care home in Markdale

The MLTC is reviewing a proposal from the Corporation of the County of Grey (the operator) for issuing an amended long-term care (LTC) home approval for Grey Gables Home for the Aged (the home), a 66-bed long-term care home located at 206 Toronto Street South in Markdale, Ontario.

  • the upgrade of the home’s 66 along with the development of 62 additional long-term care beds approved by the Ministry of Long-Term Care (the ministry), subject to project approval and meeting all licensing requirements under the Long-Term Care Homes Act, 2007 (LTCHA)

The MLTC is reviewing a proposal a licence transfer request from CVH (No. 2) LP by its general partners, Southbridge Health Care GP Inc. and Southbridge Care Homes (the licensee) to transfer the licence of Birchwood Terrace (the home), a 96-bed, structurally classified as “C” long-term care (LTC) home, located at 237 Lakeview Drive in Kenora, Ontario to Wiigwas Elder and Senior Care a new non-profit organization created by Kenora Chiefs Advisory.
