Renew, replace or change an accessible parking permit
If you have an existing accessible parking permit, you can renew, replace or change information on your permit.

Overview
You can renew your accessible parking permit (APP) if you:
- have a health condition that meets the required criteria
- are a company or organization that owns or leases vehicles primarily to transport individuals with accessibility needs on a non-profit basis.
You can also get an accessible parking permit if you have a permanent or temporary need for a permit.
If you have a permanent condition
If your healthcare practitioner has certified that the disability is permanent, you do not need to be re-certified by your healthcare practitioner. Renew online or complete only Part A of the application (if renewing by mail or in-person).
If you have a temporary condition
If your disability is temporary or subject to change and your accessible parking permit has expired, your healthcare practitioner must re-certify your health condition before you can apply for another permit. This means you must complete Part A of the application form and a healthcare practitioner must complete Part B of the application form.
Using the permit
An individual accessible parking permit is issued to a person, not a vehicle. In order to park in an accessible parking space, you must display your permit on the dashboard or sun visor of the car you are travelling in, so the permit number and expiry date is clearly visible.
Only the person named on the permit can use it to park in an accessible parking space. If a person who is not identified on the permit uses the permit without the permit holder in the vehicle, it can result in fines of up to $5,000 and the permit may be seized.

There is no cost to renew an accessible parking permit.

Renew online
You can renew your accessible parking permit online (if applicable), by mail or at a ServiceOntario centre.
Eligibility
You may renew online if you:
- have an existing permanent permit and do not need recertification from a healthcare practitioner
- understand and consent to ServiceOntario verifying the information provided in the application form to determine whether to issue or renew the accessible parking permit
To renew online, you will need the:
- permit holder’s given legal names (first, middle and last), date of birth, gender, mailing and residential address, phone number and email address
- current accessible parking permit number
- permit holder’s driver’s licence number (if applicable)

If you are renewing your subject to change or permanent accessible parking permit by mail, send copies of your identification proving legal name, date of birth, and signature. Do not send original documents. Check the full list of acceptable identification documents.

Renew in person at a ServiceOntario centre
If you cannot renew online or by mail, bring your completed application with identity documents to a ServiceOntario centre.

Renew for someone else
If applying on behalf of an adult incapable of applying for a permit, you will need to submit one of the following:
- a Power of Attorney document which authorizes you to act on their behalf
- a court appointed guardianship proof of Ontario Disability Support Program (ODSP) trustee status
- a Sworn Statement signed by a Commissioner of Oaths (not applicable to dependents under the age of 18)
The authorized representative must also provide their own identification documents with their legal name, date of birth, and signature.
Interim permit
If you require a permit immediately, you may visit a ServiceOntario centre for an interim accessible parking permit that will be valid for up to 120 days.

Renew a company vehicle accessible parking permit online
An accessible parking permit may be issued by companies and organizations for vehicles that transport individuals with disabilities, provided that they meet the eligibility criteria. You can get a company accessible parking permit for every vehicle that your organization owns or leases to provide transportation services to individuals with disabilities.
It is valid for 5 years and, if still required, must be renewed every 5 years.
To be eligible:
- your company or organization must be registered and operating in Ontario
- vehicles must be registered to your company or organization
- vehicles must be owned or leased by the company or organization primarily to provide transportation services to persons with a disability on a non-profit basis

Renew company accessible parking permit
Acceptable identification
To renew an accessible parking permit, you must provide proof of the following three elements of your identity:
- legal name
- date of birth
- signature
If applying by mail, send copies of your identification documents only. Do not send original identity documents.

- Passport – Canadian or Foreign (not expired more than one year)
- Canadian Citizenship Card with photo (issued prior to February 1, 2012)
- Canadian Permanent Resident Card with signature
- Record of Landing (Form IMM 1000)
- Confirmation of Permanent Resident (Form IMM 5292)
- Report Pursuant to the Immigration and Refugee Protection Act with photo (Form IMM 1442)
- Student Authorization/Study Permit (Form IMM 1442)
- Employment Authorization/Work Permit (Form IMM 1442)
- Visitor Record (Form IMM 1442)
- Temporary Resident’s Permit (Form IMM 1442) – formally Minister’s Permit/Extension of Minister’s Permit
- Secure Certificate of Indian Status Card (issued on or after December 15, 2009, by Indigenous and Northern Affairs Canada)
- Driver’s licence, Enhanced driver’s licence or Ontario Photo Card – issued by the Government of Ontario (not expired more than one year)
- Canadian Department of National Defence driver’s licence (Form DND 404)
Documents that can be combined to provide proof of all three elements of identity (legal name, date of birth and signature) when shown together

- Canadian Birth Certificate or a proof of birth document issued by a US jurisdiction
- Canadian Permanent Resident Card without signature (issued after February 4, 2012)
- Certified copy of Statement of Live Birth – issued by the Government of Ontario
- US Passport Card
Proof of legal name
- Certified copy of marriage certificate – issued by the Government of Ontario
Proof of legal name and signature
- Certificate of Indian Status (laminated card issued by Indigenous and Northern Affairs Canada)
Proof of date of birth
- Nexus Card and FAST/EXPRESS Card
Proof of signature
- Driver’s licence or enhanced driver’s licence – issued by another Canadian or US jurisdiction
- Ontario Health Card
- Identity card issued by a Canadian or US jurisdiction (enhanced or non-enhanced)
- Ontario Student Card with signature
- Declaration from a Guarantor (Form SR-LD-40)
- Canadian Department of National Defence (DND) Identity Card
Authorized representatives
If an authorized representative is renewing on your behalf, you must sign the application form and provide copies of your identity documents. Your identity documents must prove your legal name, date of birth and signature.
If you are unable to sign the application form and an authorized representative has signed the application form on your behalf (not applicable to dependents under the age of 18), the application must be accompanied by one of the following:
- a Power of Attorney document which authorizes them to act on your behalf
- court appointed guardianship
- proof of Ontario Disability Support Program (ODSP) trustee status
- a Sworn Statement signed by a Commissioner of Oaths

Change permit information
You must update your accessible parking permit when:
- you change your name
- you change your address
- your medical condition changes
- other personal information changes (for example, gender)
Note: You may now apply for a change of address online.
For all other permit changes, you can get an application form at any ServiceOntario centre or download the accessible parking permit form. Submit the form in person at a ServiceOntario centre or by mail to:

Note: If you are changing your name or other personal information, you must also provide an identity document showing proof of new name.
Return a permit
The permit must be returned to ServiceOntario within 30 days when the:
- person named on the permit is deceased
- permit holder is no longer a person with an eligible health condition
- permit contains incorrect information
- permit has been cancelled

Damaged, lost or stolen accessible parking permit
To replace a damaged or lost accessible parking permit (in person or mail in only):
- Get an application form at any ServiceOntario centre or download the accessible parking permit form. Your healthcare practitioner is not required to complete Part B.
- Provide proof of the following: legal name, date of birth, and signature. If applying by mail, send only photocopies of your identity document. Do not send original identity documents.
- Submit the form (and the damaged accessible parking permit, if applicable) to a ServiceOntario centre in person, or by mail to:

Note: If you can’t return the damaged permit, you must apply for a lost accessible parking permit instead.
To replace a stolen accessible parking permit (in person or mail in only):
- Get an application form at any ServiceOntario centre or download the accessible parking permit form. Your healthcare practitioner is not required to complete Part B.
- Provide proof of the following: legal name, date of birth, and signature. If applying by mail, send only photocopies of your identity document. Do not send original identity documents.
- Report the stolen permit to the police by visiting or calling your local police services to report your stolen accessible parking permit. You will need to obtain an occurrence number and write it on the application form.
- Not all police departments participate in telephone reporting. Check with your local police to determine how to report a stolen permit.
- Submit the application form to a ServiceOntario Centre in person, or by mail to:

Seized permit
If your accessible parking permit has been seized, you cannot get a new permit until the matter has been resolved in court.
