COVID-19 Vaccination Record Requests

Requesting your digital Proof of Vaccination Credential (PVC)

Proof of vaccine for COVID-19 can now be requested and received immediately through a simple form. If you need COVID-19 vaccine proof for travel or any other reason please use that method.

You can also request a printed copy at your health centre or in Yellowknife at the pickup office in the Centre Square Mall.

If you previously requested a copy of your COVID-19 medical record proof it is still valid proof of vaccination, but it will not be able to be scanned and verified because it does not have QR code.

At this time, the GNWT is not issuing electronic Proof of Vaccination Credentials (PVC) for people aged 5 to 11 years. If you require proof of vaccination for a dependent 5 to 11 years old, you can request your personal health information.

Difficulties with the online PVC Form:

If you are having difficulties downloading your PVC, please email NTHSSA_covidvaxrecords@gov.nt.ca and include:

  • Scanned copy of your NWT Healthcare Card – must be valid and not expired
  • Scanned copy of your government-issued photo ID
  • Any out-of-territory documents or record of immunization, in order to validate, that include:
    • First and last name
    • Date of vaccination
    • Community and Province/Territory of vaccination
    • Vaccine name and lot number
    • Size of dose administered
    • Where on your body you received the vaccination

Adding an out of territory dose to your  NWT Proof of Vaccination Credential (PVC):
