Register for a Family Doctor or Nurse Practitioner

You should add your name to the Provincial Patient Registry and keep your information up to date to ensure a family doctor or nurse practitioner will be assigned to you when one becomes available. You can use this online form if you:

  • do not have a family doctor or nurse practitioner and would like to register for one;
  • have moved within the province and are looking for a family doctor or nurse practitioner in your new area; 
  • are expecting a baby and do not have a family physician or nurse practitioner; or
  • are currently on the Patient Registry and your contact information has changed.

What will I need to complete the form?

You will need your PEI Health Card number to register. If you are registering a household, you will need PEI Health Card numbers for all members.

How long will it take to complete the form?

It will take a maximum of 15 minutes to complete the form, depending on how many people you are registering. 

Patients are assigned in chronological order, so those on the registry the longest are assigned first. You will be contacted by the Patient Registry when a family doctor or nurse practitioner is accepting new patients in your area.
