QUESTIONS ON DC-01 (January 2002)

(Q) May we submit applications for equipment certification using the older
method?

(A) No. The procedure CP-01 has been superseded by DC-01. We will no
longer accept applications for certification. It is now a registration process
.
Note: Radio Apparatus still requires equipment certification.

(Q) How do I obtain a Company Number – is there a fee for this?

(A) To request a company number email us at certification.bureau@ic.gc.ca
with your full mailing address, including contact name and email
address. You should request this number before filing. There is no
charge for the number.

    (Q) Does IC choose the Unique Product Number (UPN)?

    (A) It’s preferable that the client choose the UPN.


    (Q) What is the reference and agreement number in DC-01?

    (A) These numbers are the client’s reference numbers used in Appendix I
    and II.


    (Q) On the Registration Request at the top right it asks for a DoC/DDC #
    - what is this?

    (A) It is the client’s reference number – the same number used for Appendix I.


    (Q) What do you mean by Canadian representative?

    (A) This could be the supplier or importer – someone that can fulfill the
    requirements in DC-01 Part 7.2, capable of responding to enquiries and
    providing audit samples at no charge to the Department if requested.


    (Q) Do I need a Canadian representative for equipment prior to January
    19th, 2002?


    (A) The requirement for a Canadian representative applies to the registration
    of equipment after January 19th, 2002.


    (Q) DC-01 says nothing about family of models?

    (A) A family of models can have the same registration number if they are
    electrically identical.


    (Q) How do we handle multiple listings?

    (A) If equipment was previously registered by "company A" and "company B"
    wants to sell this same equipment using “company B’s” name and model
    number, a new registration number must be issued as the company number
    would change. In all cases, registration and DoC is required for each and
    every model separately.


    (Q) What is required if we modify our Registered equipment?

    (A) The equipment must be tested to CS-03 by a test facility that is accredited
    and recognized by Industry Canada. You must retain the test results for
    audit purposes. Registration must be made using DC-01 only if the
    information in Appendix II has changed. There is no charge for this service.


    (Q) What is required if we modify certified equipment under CP-01?

    (A) The equipment must be tested to CS-03 by a test facility that is accredited
    and recognized by Industry Canada. You retain the test results for audit
    purposes. Register using DC-01. The registration number will be the same
    as your previous certification number. Submit Appendix I and II of DC-01.
    It will be retained on file. There is no charge for this service.


    (Q) Is the registration fee taxable?

    (A)No.

    (Q) If there is more than one model in a family, can all models be submitted in
    one package or does each model require a separate DoC and Registration
    Request?

    (A) They may all be submitted on one registration form but there is a $45.00
    fee for each model listed.

    SELF MARKING Q & A’s

    (Q) Can I still buy labels?

    (A) Under DoC all new equipment must be marked in accordance with the
    self-marking procedure. For equipment certified prior to January 19th,
    2002 labels may be ordered until January 2003. Only self-marking will
    be offered after that date
    .

    (Q) Under the self-marking agreement do I have to pay for all marks or only
    Canadian shipments?


    (A) Yes, you must pay for all marks.

    (Q) What is the penalty for not conforming to the self-marking payment request?

    (A) The offender, individual or corporation may suffer loss of registration and is
    liable to legal action as per Section 73.2 of the Telecommunications Act.


    (Q) Can the forms for self-marking be submitted electronically?

    (A) Yes, you may forward your request by email or fax. If using email, you
    must ensure that you can provide an electronic signature for both the TAC
    Holder portion and the credit card authorization form.


    (Q) Where may I obtain numbers as referenced in the Self Marking Agreement?

    (A) The Agreement Number is assigned by Industry Canada. The Request
    Number may be your reference number.


    (Q) May a company register new equipment and apply for self-marking
    simultaneously? Are two methods of payment required?


    (A) Yes, you may apply simultaneously, however, the appropriate
    Self-Marking Agreement forms must be submitted separately with
    individual payments.


    (Q) May a Test Lab apply for Self-Marking on behalf of his client?

    (A) Yes, if the laboratory submits a signed letter of authorization from the client.

    (Q) What is a UPN?

    (A) This is the Unique Product Number which is assigned by the declaring
    party for registration of DoC. Example: IC: xxxxxx-yyyyyyyy. The x
    portion is your company number. This company number includes an alpha
    character which indicates company location. The y portion is a unique
    product number (UPN) chosen by your company.


    (Q) How is the six-month marking verification to be done?

    (A) The TAC holder or Declaring Party must submit documentation to the
    Department rendered accurate and verified by someone of authority
    (ie. Accountant, CEO) indicating units marked against units paid for.


    (Q) Do taxes apply to Self-Marking fees?

    (A) If the address on the agreement form is a Canadian address, the GST
    and/or PST apply.


    (Q) Is there still a $5000.00 limit when paying by credit card.

    (A) Yes. There is a transaction limit of $5000.00.
