Civil Aviation Daily Occurrence Reporting
System Program Follow-up Reporting

Policy Objective

To establish occurrence reporting follow-up action and procedures in a consistent national approach in order to enable Transport Canada Civil Aviation to seek out safety deficiencies, take appropriate corrective action, conduct trend analysis and develop preventive safety measures.

Background

The Civil Aviation Daily Occurrence Reporting System (CADORS) reports receive national distribution to Civil Aviation safety inspectors and provides timely information on daily operational occurrences within the National Civil Air Transportation System (NCATS).

The program is managed by Regional System Safety offices who receive and gather the information from both external and internal sources. System Safety personnel record the information in a database and a CADORS number is generated for tracking.

When an occurrence is being investigated by the Transportation Safety Board (TSB), the Regional Director appoints a Minister's Observer in accordance with the Minister's Observer Manual (TP11776). When the TSB is not investigating, the Regional Director determines what action shall be taken by Civil Aviation. In addition, there are a number of other occurrences, which constitute a CADORS report and are recorded in the CADORS system. The Regional Director determines what follow-up action is required.

CADORS Manual (TP4044), Annex A defines the events recorded for each occurrence.

Occurrence reporting follow-up action and procedures are required in order that Civil Aviation fulfill its safety mandate.

Policy Statement

CADORS and follow-up reports enable Civil Aviation to seek out safety deficiencies, take appropriate corrective action, conduct trend analysis and assist in developing preventive safety measures.

Application

This directive applies to all areas of operation within Civil Aviation.

Definition

For the purpose of this directive, CADORS reportable occurrences are defined in ANNEX A.

Approach

Regional and Headquarters Directors shall implement a Civil Aviation Daily Occurrence Reporting System follow-up program and will ensure that findings and actions taken are included in CADORS follow-up reports.

Enforcement actions resulting from a regulatory investigation will not be recorded in the CADORS follow-up report.

Reporting

System Safety will maintain contact with the division or branch of primary interest for follow-up information. The branch of primary interest will ensure that information uncovered and action taken is passed to System Safety to be entered into the national CADORS database. Regional and Headquarters reports will be produced from the data.
