Forms and publications

All citizens require the Request for certificate or copy of act form to request civil status documents such as a certificate or copy of an act of birth, marriage, civil union or death. In December 2005, three types of Request for certificate and copy of act forms were introduced, namely one for each life event:

Birth
Marriage or civil union
Death

These documents are available in electronic format. They are downloadable in PDF (Portable Document Format) version. They can be consulted with Adobe Reader software that you can download free of charge. Dynamic PDF forms can be viewed, filled out on screen and printed. Static PDF forms can only be printed.

Please note that the Request for certificate document in the form of a green leaflet is no longer accepted. However, the Request for certificate or copy of act form covering the four types of events (birth, marriage, civil union, death) is still accepted.
