Applying for a travel document

Personal information

All your identification and travel arrangement documents, including your travel document, should be issued under the same name. Otherwise, you may encounter problems and delays at border crossings.

To obtain a travel document in your new name after a legal change of name, submit one of the following:

  • the legal change of name document, or
  • the adoption order, or
  • approved official ammendment from Citizenship and Immigration Canada. (IMM 1436)

Date of birth

Your date of birth will be inscribed in the travel document exactly as it appears on the proof of immigration status unless Citizenship and Immigration Canada records have been amended subsequent to your admission to Canada. If this is the case, the original approved "Request to Amend Immigration Record of Landing" form IMM 1436 is required. If your proof of immigration status does not show a complete date of birth, indicate the specific day, month and year of your birth. Provide copies of any documents you may have showing this date of birth.
