Prescribed RL-16 slip

You can obtain the RL-16 slip at any Revenu Québec office or through the service Ordering forms and publications.

Unofficial slips or facsimiles

You may file unofficial RL slips or facsimiles. To obtain authorization to do so, submit two original copies of your RL slips to the following address:

Please note that Revenu Québec offers no monetary compensation to persons who provide their own slips.

Filing the RL-16 slip

You must attach copy 1 of each RL-16 slip to the trust income tax return.
