Giving Voices to Seniors and Veterans

Victorian Order of Nurses (VON) Canada is presently conducting a survey. Its goal is to take action and change the face of home and community health care for seniors and Veterans. Please make a difference and participate in this short survey. The information and answers you will provide will be held in strict confidentiality.

In order to answer the questionnaire, you will need to save it on you PC. The questionnaire is available in two (2) formats: PDF (Portable Document Format) and RTF (Rich Text Format).

For PDF files, you will need to use the Adobe Acrobat Reader. If you do not have access, you can download a free copy from Adobe.
