Claim Form (80kb PDF):
Employees who believe their employers did not pay them correctly, can file a claim with the Employment Standards Branch. Claims must be filed within six months of the last day of work or when the incident happened. Employment standards staff can only investigate and collect up to six months of unpaid and overtime wages and up to 22 months for vacations and general holiday pay.

Employment Permit Application for Young Employees Form (104kb PDF):
The application requires information from the applicant, a parent/guardian, the school principal and the employer. The completed form can be taken, mailed, or faxed to the nearest Employment Standards Branch office.

Averaging Permit/Order Application Form (52 kb PDF)
Employers can apply for a permit or order to: average the standard hours of work across a longer period; reduce the 30 minute break period; or change the day of rest. A request generally needs the written agreement of the majority of the employees who will be affected by the permit/order.
