How to obtain a certificate or copy of an act?

    You must obtain the form that meets your need, namely:

    Birth
    Marriage or civil union
    Death

    The authorized forms are available in electonic or paper version.

    Attention: In order to protect your identity, we remind you that a certificate or copy of an act may be delivered only to the person whose name appears on the act used to enter the event in the register. If the requester is another person, the latter must justify his or her interest to the Directeur de l'état civil.

  2. Select the type of processing and document

    Before presenting a request, the requester is responsible for learning about the type and format of the document sought from the agency to which it must be presented.

    When the request concerns the birth of a minor person, the Directeur de l'état civil recommends using the full-size birth certificate format since it mentions the names of the parents.

    The Directeur de l'état civil may handle your request on a normal or accelerated basis. The cost, processing time and delivery method varies depending on the type of service chosen. The fee covers processing the request, the printed document and shipping fees.
     

    Normal processing service
    Processing time: 3 weeks or more
    Delivery by ordinary mail

    $15 per certificate
    $20 per copy of an act
     

    Accelerate processing service
    Processing time: 6 business days or more
    Delivery by Xpresspost in Canada
    Delivery by ordinary mail outside Canada

    $35 per certificate
    $35 per copy of an act

  3. Send the signed form together with photocopies of identity documents and payment

    Do not forget to:

    sign your request
    include your payment (see table above)
    include photocopies of two documents to establish your identity (see the list of main documents accepted).

    To avoid delays in processing your request, make sure that photocopies of documents are legible.

    If you cannot furnish a document containing a photograph, you must turn to an authorized respondent who has known you for at least two years. To do so, use the form Guarantor's declaration

    If you fax your request, the documents may not be clear and your ID may be illegible. If this occurs, we will be unable to process your request and additional delays must be anticipated.
