Home Care Forms
Home Care has traditionally provided the following forms in hard
copy. These provincial forms are now available electronically and
are intended for use by Regional Health Authority (RHA) Home Care
professional staff to document confidential information intended for
care planning, service delivery and evaluation by the RHA. The
information which will be collected is private and legally protected
by law. It is not to be transmitted or distributed to any source not
previously authorized by the RHA. Should anyone receive a
completed form in error, please notify the RHA
immediately by telephone or contact the RHA main office by email.
These forms will be reviewed and updated periodically.
Manitoba Health is exploring the “Fill & Save” option on the
forms.

The forms included in this
site have been created in Portable Document Format (PDF).
In order to view and print these forms, you will need the
free Adobe Acrobat Reader. If you do not already have this
on your computer, it is available free of charge, from
Adobe Systems Incorporated. PDF documents function on many
different platforms including DOS, Windows, Mac, Unix, and
Linux.
If you already have Adobe Acrobat Reader on your computer,
you need only click on the Download link and the
form will appear on your screen. You can then print it on
your printer and fill it out as you would any paper form.
To save the blank form on your computer for future use,
click on File in your browser's menu bar, then on Save as.
You may then select the drive (usually your C drive) and
directory where you wish to store the form. You may also
change the name of the file that appears in the File name
area, but you must be sure that .pdf follows the name.
If you are not sure whether or not Adobe Acrobat Reader is
already installed on your computer, simply click on the
Download link. If the form does not appear on your
screen, then the necessary software is not present on your
system.
This site has been designed for use by the widest possible
audience. There may be however, some system configurations
that conflict with the PDF download process. If you
experience difficulty downloading a form from this site,
the following procedure may allow you to use our forms
successfully.
- Click the right mouse button once on the
Download link.
- From the menu that appears, choose the option
Save Link As (wording may vary slightly depending
on the browser).
- Give the form a name and save it in a folder or
other location of your choice on your computer.
- Go to your Start Programs and open Adobe
Acrobat Reader.
- Choose File, then Open and double
click on the form you named and saved, and the form
will open.
