grants advance approval to Telmax's configuration subject to the provision of another
affidavit after the installation of its first services affirming that they are configured
to be compliant with the requirements for an exemption.

telephone network connectivity, and Digital Exchange Access service. Eff.

rates for customer-specific business local access service and to incorporate the Greater
Vancouver surcharges, approved in Telecom Order CRTC 99-1134 dated 9 December 1999, in the rates for this

TCBC's request to restructure the rates for business individual line service, business
multine service, and for the local business contract option, and to incorporate the
