Seniors' Care : Arranging Care

There are three aspects to arranging care that are important to remember :

  • Determining your eligibility (or whether you qualify for a particular service or subsidy)
  • Selecting the organization(s) that will provide the service (a "provider organization")
  • Applying for care (filling in the appropriate paperwork)

This section describes the aspects of arranging care for yourself or a member of your family once you know the type of care you are looking for. For more information about the types of care available, please visit the Home and Community Support Services and the Residential Care sections of this site.

Speak to your CCAC

Your local Community Care Access Centre (CCAC) can help you with defining your needs and personal situation, determining your eligibility, locating providers and applying for care. A CCAC can also provide information about the availability of financial subsidies for particular service options.

While the extent of CCAC involvement in arranging care can vary with each option, they can always connect you to the local service providers and answer your questions during the steps in the process of arranging care.

Home and Community Support Services

Contacting your CCAC to find out whether you are eligible for the government-funded home and community support services is your first step. If you are, then as the following chart shows, the CCAC co-ordinates your application, and selects the provider for you.

If you are not eligible for the government-funded services arranged by the CCAC, or you wish to make your own arrangements (or supplement the CCAC services) you will play a more direct role in arranging care.

You would do this by contacting the provider organizations to find out about your eligibility and apply for care. For more information, see the Finding a Provider page.

Residential Care Settings

Unlike Home and Community Support Services where eligibility relates to government funded services, the term eligibility in this section relates to whether you qualify for one of the residential care settings.

If you are eligible for admission to a long-term care home, then the CCAC co-ordinates your application for you and you select your top three choices of Homes.

For supportive housing and retirement homes, you will need to apply directly to the provider and confirm your eligibility to the particular provider. The CCAC can provide you with lists of providers in your area, and information about these options.

Long-Term Care Homes You select a maximum of 3 providers (except for short-stay respite or if immediate admission is required). The CCAC co-ordinates your application.
